1. Click on Dispatch

2. Select Enter Status Codes

3. Click on the Status: Complete and then click Edit

4. Select the Notification tab

5. Click the Hyperlink button

6. Type in "Click here for our survey!" and then paste the survey link into the URL field and click OK.

7. Your survey is now ready to go.
